This page allows you to send a non-urgent request for a prescription refill to your UCLA pharmacy. Your medication is filled at UCLA pharmacies if it has a "Prescription #" at the end of the medication description. Enter any additional comments or instructions in the box provided.

If the prescription has no remaining refills, is expired or is owned by a non-UCLA pharmacy, a renewal request will be sent to your doctor’s office.
