    TERMS AND CONDITIONS OF USE

    myUCLAhealth

    UCLA Health is pleased to offer patients the ability to view their medical information and interact with physicians’ offices online by using myUCLAhealth. This web site and related services are subject to compliance with the terms and conditions set forth below. Please read the following information carefully.

    For any medical emergency, call 911 or go to your nearest emergency room.

    BY USING MYUCLAHEALTH, YOU SIGNIFY YOUR AGREEMENT TO THESE TERMS AND CONDITIONS. IF YOU DO NOT AGREE TO THESE TERMS AND CONDITIONS, YOU ARE NOT ABLE TO USE MYUCLAHEALTH.

    TERMS AND CONDITIONS

    1. myUCLAhealth Capabilities

    UCLA Health provides myUCLAhealth to allow UCLA Health patients and appointed patient representatives to view health-related information, communicate with UCLA Health practitioners and staff, and arrange for clinical and health plan services, and access additional services.

    The information provided on myUCLAhealth is not a substitute for the advice of a personal physician or other qualified healthcare professional. Always seek the advice of a physician or other qualified healthcare professional with any questions you may have regarding medical symptoms or a medical condition. Never disregard professional medical advice or delay in seeking it because of something you have read on myUCLAhealth.

    For urgent questions, please call your doctor's office.

    2. Electronic Communications

    All communications between you and your UCLA Health care team using myUCLAhealth are carried over a secure, encrypted connection directly into the UCLA Health electronic medical record. While you may receive internet e-mail messages notifying you of new messages in your myUCLAhealth Inbox, these e-mails will not contain any confidential medical information.

    Messages that you send via myUCLAhealth will become part of your permanent legal medical record. Once a message is made part of your permanent medical record, it will be accessible to current and future UCLA Health care provider(s) and staff members at UCLA Health who are appropriately involved in your care.

    Your health care provider may choose to select an associate(s) to help manage and respond to your electronic messages on myUCLAhealth. These may include nursing or other administrative staff who review messages sent via myUCLAhealth and either respond or route the message to the health care provider. Use of myUCLAhealth secure messaging may not be appropriate if the message includes sensitive information that you would only wish to discuss directly with your provider.

    UCLA Health will make its best effort to provide a timely response to electronic inquiries or other communication submitted through myUCLAhealth. In some cases, UCLA Health staff may not be immediately available, so you should allow at least two (2) business days for a response. Emergency matters requiring immediate attention should not be submitted through myUCLAhealth.

    With respect to any electronic communications sent through myUCLAhealth, UCLA Health is only able to respond to such communications based on the information provided by the patient. If insufficient information is provided, UCLA Health will be unable to provide accurate and reliable services.

    3. Eligibility to Use myUCLAhealth

    Two requirements must be met for a patient to be eligible to use myUCLAhealth. First, a patient must be twelve (12) years of age or older. A minor under the age of 12 years may not establish a myUCLAhealth account. Second, a patient must be an active patient of UCLA Health healthcare provider.

    4. Access to Family Health Information on myUCLAhealth

    A. Adults 18 years of age or older

    Adult patients age 18 years of age or older, may grant proxy access to any adult by submitting the appropriate form.

    B. Minors Between the Ages of 12 and 17

    California State law and Federal law prohibits a parent or legal guardian from accessing certain types of medical information about a minor age 12 or older without the specific consent of the minor. Due to these legal requirements, full access to myUCLAhealth is available for patients between the ages of 12 and 17, but only limited access is available to the patient’s parents or legal guardians.

    C. Minors Under Age 12

    Only parents or legal guardians of minor children under the age of 12 may request access to the minor child’s information available through myUCLAhealth. In such cases, the party requesting access must demonstrate the legal right to that access.

    If you have been granted access to a minor patient’s record on myUCLAhealth and your legal relationship with the patient changes, you must inform UCLA Health immediately. Please call Health Information Management Services (HIMS) at (310) 825-6021. UCLA Health reserves the right to revoke access to a patient’s myUCLAhealth record at any time for any reason.

    5. User Participation in myUCLAhealth

    Although myUCLAhealth displays certain information from your health record, it does not necessarily display all information found in your medical record. If you think that your health information displayed in myUCLAhealth is inaccurate, you can request that UCLA Health amend your myUCLAhealth by messaging your provider(s) within myUCLAhealth.

    To request a complete copy of your medical records, please contact: Health Information Management Services (HIMS) at (310) 825-6021.

    You agree that you will not upload or transmit any communications or content of any type (including secure messaging) that may infringe upon, misappropriated, or violate any rights of any party. In consideration of being allowed to use myUCLAhealth, you agree that the following actions shall constitute a material breach of these Terms and Conditions:

    UCLA Health expressly reserves the right, in its sole discretion, to terminate or limit a user’s account due to act delineated above, any other act that would constitue a violation of these Terms and Conditions, or at the discretion of the patient's physician or practice manager. UCLA Health may limit a user's myUCLAhealth account by enabling "view only" access or terminating access to interactive services and/or any or all other areas of myUCLAhealth.

    You may deactivate your MyUCLAhealth account at any time. To deactivate your myUCLAhealth account, please contact your physician’s practice or contact Health Information Management Services (HIMS) at (310) 825-6021. Your myUCLAhealth account will automatically be deactivated upon death.

    6. Customer Service Information

    7. Security and Confidentiality

    UCLA Health is committed to protecting the confidentiality of your medical information. Any personal information you submit or that is maintained on myUCLAhealth is governed by UCLA Health’s privacy and security practices. Firewalls, passwords, encryption, and audit trails are further used to safeguard your information. UCLA Health has taken steps to make myUCLAhealth secure against unauthorized access and use. This web site uses encryption software. While UCLA Health uses state-of-the-art security, no system can perfectly guard against risks of intentional intrusion or inadvertent disclosure of information. When using myUCLAhealth, information will be transmitted over a medium that is beyond the control of UCLA Health and its contractors. YOU HEREBY EXPRESSLY ASSUME THE SOLE RISK OF ANY UNAUTHORIZED DISCLOSURE OR INTENTIONAL INTRUSION, OR OF ANY DELAY, FAILURE, INTERRUPTION OR CORRUPTION OF DATA OR OTHER INFORMATION TRANSMITTED IN CONNECTION WITH THE USER OF THIS SERVICE.

    UCLA Health affords the same degree of confidentiality to medical information stored on myUCLAhealth as is given to information stored by UCLA Health in any other medium. UCLA Health limits employees’ access and ability to enter or view health information based upon their role in your care. UCLA Health has taken steps to make sure all health information received from online visitors is as secure as reasonably possible against unauthorized use, access or disclosure.

    UCLA Health and many other organizations and individuals such as physicians, hospitals and health plans are required by law to keep your health information private and confidential. If you have authorized the disclosure of your health information or your family’s health information to someone who is not legally required to keep it private and confidential, it may no longer be protected by state or federal privacy and confidentiality laws.

    Your privacy is important to us. UCLA Health will use your confidential medical information in order to provide you health care services. UCLA Health takes many precautions to maintain your confidential medical information in confidence and will not deliberately disclose any information regarding you to any unaffiliated third party unless you authorize that person to receive your information or the information is permitted to be disclosed by law. Please review the UCLA Health Notice of Privacy Practices (http://www.uclahealth.org/privacy) for a thorough description of how UCLA Health gathers, uses and protects your confidential information. All messages sent and received within myUCLAhealth that contain medical information are subject to all state and federal laws governing the security and confidentiality of medical records.

    8. Passwords

    UCLA Health has several tools that allow you to record and store information. You are responsible for taking all reasonable steps to ensure that no unauthorized person shall have access to your UCLA Health online password or account. It is your sole responsibility to (1) control the disclosure and use of your activation codes, username and password; (2) authorize, monitor and control access to and use of your myUCLAhealth account and password; (3) promptly change your password in myUCLAhealth is it has been compromised; and (4) promptly inform the Health Information Management department at UCLA Health of any need to deactivate a password. UCLA Health recommends that you change your password on a regular basis.

Unless specifically authorized in writing by UCLA Health, any use of these materials, or of any materials contributed to myUCLAhealth by entities other than UCLA Health, on any other Web site or networked computer environment for any purpose is prohibited.

Nothing on this web site should be construed as granting any license or right to use any trademark, service mark, or logo displayed on this website without the express written permission of UCLA Health or the third party that may own the trademark, service mark or logo.

    UCLA Health grants a limited license to each user to make personal use only of this web site and the associated services in accordance with these terms and conditions of use.
